The Complete Guide to ADHD Medication

The Two Categories: Stimulants and Nonstimulants
ADHD medications fall into two broad categories: stimulants and nonstimulants. Stimulants are considered the first-line treatment for ADHD in most children, adolescents, and adults. Name-brand medications such as Concerta, Ritalin, and Focalin are methylphenidate-based medications, meaning they work by blocking the reuptake of dopamine and norepinephrine in the brain. Alternatively, Adderall, Vyvanse, and Dexedrine are amphetamine-based medications, which work by both blocking reuptake and increasing the release of dopamine and norepinephrine, which can make them more potent. Both work quickly–stimulant medications can produce effects within 30 to 60 minutes after ingestion. These medications are available in short-acting and long-acting forms, so that individuals may utilize stimulants across a school or work day.
Nonstimulants are used alternatively for several reasons, including when stimulants aren’t well-tolerated or as effective as expected. Nonstimulants can also be used alongside a stimulant to address symptoms that aren’t fully covered. There are currently 4 nonstimulants that have FDA approval for ADHD: atomoxetine (i.e., Strattera), viloxazine (i.e., Qelbree), guanfacine extended-release (i.e., Intuniv), and clonidine extended-release (i.e., Kapvay). The timeline of nonstimulants can be similar to other forms of psychotropic medications, such as Selective Serotonin Reuptake Inhibitors (SSRIs), taking up to several weeks to reach full effect.
Factors to Consider When Choosing ADHD Medication

Age and developmental stage: some medications have better safety and efficacy data in specific age groups (e.g., guanfacine and clonidine have only been FDA-approved for children and adolescents ages 6 to 17).
Coexisting conditions: a history of anxiety, tics, cardiovascular concerns, or substance use may guide treatment planning away from medications known not to exacerbate these conditions.
Duration of coverage needed: the amount of time an individual wants symptom control matters–this informs whether a short-acting or long-acting medication may be better suited.
Prior response and side effects: sometimes methylphenidate-based medications may work better for someone than amphetamine-based medications, and vice versa.
Misuse risk: if a personal or family history of substance use is present, a nonstimulant or a stimulant with a lower misuse risk may be more appropriate.
Side Effects and Monitoring

Every ADHD medication has potential side effects, and part of a prescriber’s job is to monitor for these. The most common side effects for stimulants are:
- Reduced appetite
- Difficulty falling asleep
- Increased heart rate
- Mild increases in blood pressure
Serious cardiovascular complications are possible but rare, which is why prescribers typically monitor heart rate and blood pressure throughout treatment.
The side effects for nonstimulants include (by medication):
- Atomoxetine and viloxazine: fatigue, nausea, mood changes.
- Guanfacine and clonidine: drowsiness, sudden drop in blood pressure.
Guanfacine and clonidine are also used to treat high blood pressure, which is why they can cause a sudden drop upon intake. As a result, these medications are usually begun at a low level and adjusted upward very gradually.
Rule of Thumb: Any new or worsening mood symptoms, chest pain, fainting, or significant changes in sleep or appetite are worth contacting your provider about quickly instead of waiting for your next scheduled appointment.
Beyond Medication
Medication can meaningfully reduce core symptoms of ADHD and reduce the severity of functional impairment. However, most comprehensive treatment plans combine medication with supports such as therapy and behavioral coaching to address factors medication alone can’t treat. This is especially important for people who were diagnosed later in life or have to unlearn unhealthy coping patterns to address accumulating effects. These supports can help address anxiety, mood, or self-esteem effects; build work or study habits; and teach emotion regulation strategies.
In Summary
ADHD medication isn’t a one-size-fits-all. Stimulants remain the first-line option for most people, and nonstimulants offer alternatives for varying concerns. Finding the right medication for you often takes some adjustment and communication with your provider along the way. A prescriber who knows your full history and symptoms is best positioned to help you navigate the process of finding the best fit.
